Part-Time Case Manager

Job Summary: The Colorado Coalition for the Homeless is seeking a Part-Time Case Manager to provide brokerage of community resources, conduct one-on-one case management, and on-going assessment of clients progress and needs for a formerly homeless, disabled population of single adults.
Key Responsibilities:
- Provide excellent internal and external customer service, behaving in a manner that cultivates trust, respect, and teamwork.
- Assess strengths and needs of all new residents and on a regular schedule for established clients.
- Develop initial treatment/service plans for all residents and update them as required.
- Develop interventions or linkages to address resident needs.
- Provide hands-on assistance and/or training to residents to support them in maintaining their housing in a safe and sanitary manner.
Requirements:
- Bachelors degree in human services, social work, psychology, or related field preferred or equivalent experience.
- Previous direct experience with homeless population preferred, or previous experience in a related direct human service field working with underserved populations preferred.

Working Conditions:
The Part-Time Case Manager will work evenings, weekends, and on-call, providing brokerage of community resources, conduct one-on-one case management, and on-going assessment of clients progress and needs for a formerly homeless, disabled population of single adults.
