Job Summary

WellPower - Child and Family Services is seeking a qualified Clinical Case Manager I/II to join our Juvenile Justice team. As a Clinical Case Manager, you will provide comprehensive psychological and behavioral case management services to youth involved in the Juvenile Justice system.

Key Responsibilities
  • Assess, monitor, and follow up with youth's ability to meet basic needs in moving toward recovery.
  • Provide comprehensive psychosocial services, including a continuum of high and low intensity services.
  • Help youth evaluate their strengths and symptoms, and support them in setting goals.
  • Guide and instruct youth in daily living skills, such as medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, and transportation.
  • Help youth develop social skills, interests, and leisure time activities to provide a sense of participation and personal worth.
  • Help youth find and make use of appropriate educational and employment opportunities, vocational rehabilitation services, or supported work environments.
  • Offer support, assistance, consultation, and education to families, friends, landlords, employers, community agencies, and others who come into contact with youth.
  • Identify and work with potential natural support systems, such as neighborhood networks, churches, and community organizations.
Requirements
  • Bachelor's (CM I) or Master's (CM II) Degree in psychology, social work, or other related human services degree.
  • One year experience in mental health or human services preferred.
  • One year experience working with adolescents and/or young adults required.
  • Must be fully bilingual in Spanish language.
  • Valid Colorado Driver's license and a recent clean driving record.
