This position is located at one of the various Indian Health Hospitals or Clinics under the Navajo Area Office. The selected individual will provide direct pharmaceutical care for inpatient and outpatient at the Service Units and is expected to manage patients with chronic disease states, as well as providing pharmaceutical care for acute patients.
Key Responsibilities- Conducts Medical Staff authorized outpatient pharmacy clinics for examination, follow-up and treatment of specialized patients. Takes the appropriate health history, follows and adjusts the developed treatment plan and monitors the patients outcome to the prescribed treatment plan.
- Evaluates medication regimens for appropriateness, drug interactions, therapeutic duplications, non-compliance and adverse drug events.
- Counsels patients on the use, storage, cautions and side effects of all medications dispensed as well as documentation of cognitive services provided.
- Provides highly complex, critical medication treatment, monitoring, management and consultation involving risk to patients life, safety, health or well-being with all levels of allied health providers and patients.
- Provides formal and informal education and in-services to medical staff, pharmacy, and nursing staff as requested. Provides training for pharmacy residents, pharmacy students and medical students. Provides education to patients and their families about medications, common disease states and information on health promotion.
The selected individual must meet the following requirements:
- Selectee may be subject to a probationary period.
- U.S. Citizenship is required.
- Selective Service Registration is required for males born after 12/31/1959.
- This is a designated position covered by Public Law, requiring contact or control over Indian children.
- Confidential Financial Disclosure Form required.
- Measles and Rubella immunization required for selectees born after 1957 and seasonal influenza immunization is required for all staff working in Indian Health Service health care facilities.
- COVID-19 vaccination is required for all selectees prior to entrance on duty.
- The selected individual is required to obtain and maintain medical staff clinical privileges, including any licensure requirements.
- On-call hours or standby duty may be required.
- Successfully pass the E-Verify employment verification check.
To qualify for this position, the selected individual must have a doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. The selected individual must also be licensed to practice pharmacy in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
The selected individual must have a minimum of three (3) years of progressively higher-level graduate education leading to a Ph.D. degree, Ph.D., Pharm.D. or equivalent doctoral degree. The selected individual must also have a minimum of one year of professional pharmacy experience.
The selected individual must demonstrate the knowledge, skills, abilities, and competencies necessary to perform the work of the position. Pharmacy work requires knowledge of the use, clinical effects, and composition of medications, including their chemical, biological, and physical properties.
