Job Title: Ambulatory Care Manager

Calvert Internal Medicine Group is seeking a highly skilled Ambulatory Care Manager to lead our clinical team.

Job Summary:

The Ambulatory Care Manager will collaborate with practice providers to identify patients in need of care coordination and develop individual care plans with patient-specific goals and interventions.

  • Perform comprehensive patient assessments to identify needs and develop care plans.
  • Coordinate referrals to ensure seamless transitions of care.
  • Provide patient education and support to promote health and wellness.
  • Review clinical data and metrics to inform care decisions.
  • Serve as a patient liaison and advocate with local healthcare providers.
Key Responsibilities:
  • Develop and implement care coordination strategies to improve patient outcomes.
  • Collaborate with practice providers to identify areas for process improvement.
  • Lead clinical team meetings to discuss patient care and coordinate efforts.
  • Develop and maintain relationships with local healthcare providers to ensure seamless transitions of care.
  • Monitor and report on clinical metrics to inform care decisions.
Requirements:
  • Active RN licensure with the Maryland Board of Nursing.
  • At least three years of previous clinical experience.
  • Strong clinical assessment and chronic disease management skills.
  • Knowledge of population health and value-based programs preferred.
