Speech Language Pathologist - Home Health Care

Adventist HealthCare is seeking a skilled Speech Language Pathologist (CCC-SLP) to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in providing high-quality care to patients in their homes, helping them to recover from speech and language disorders.

Key Responsibilities:
  • Conduct initial examinations and evaluations to establish a diagnosis, prognosis, and plan of care for patients with speech and language disorders.
  • Develop and implement individualized therapy plans to address patient needs and goals.
  • Provide direct patient care, including speech and language therapy sessions, and coordinate with other healthcare professionals as needed.
  • Document patient progress and communicate with healthcare teams to ensure seamless care.
  • Manage schedules and caseloads to ensure efficient and effective care delivery.
Requirements:
  • Bachelor's, Master's, or Doctorate in Speech-Language Pathology from an accredited program.
  • 1 year of home health experience preferred.
  • Valid driver's license with agency insurance requirements.
  • Active American Heart Association Basic Life Support (BLS) certification.
  • Active Maryland License.
