Key Responsibilities
  • Act as a primary advocate for terminally ill patients and their families, providing essential support in various hospice environments.
  • Conduct regular assessments and maintain contact with patients and families, ensuring their needs are met with compassion and expertise.
  • Educate patients and families on disease management, hospice care principles, and end-of-life signs, while keeping the primary physician informed as necessary.
  • Oversee the comprehensive care plan for each patient, collaborating with an interdisciplinary team to ensure high-quality care delivery.
  • Document patient interactions and care plans in the electronic health record, adhering to all regulatory standards.
  • Monitor patient responses to treatments and adjust care plans as needed, communicating changes to the healthcare team.
  • Supervise home health aides and licensed practical nurses, ensuring compliance with agency standards and regulations.
  • Facilitate discharge planning and coordinate necessary resources for continued care post-discharge.
Qualifications
  • Licensure: Valid Registered Nurse license in the state of practice, along with Basic Life Support (BLS) certification.
  • Education: Completion of an accredited nursing program.
  • Experience: 1-2 years in nursing, preferably in hospice, home health, or related fields.
Skills and Abilities
  • Strong assessment and critical thinking skills to identify patient needs and implement appropriate interventions.
  • Excellent communication and interpersonal skills to foster relationships with patients, families, and team members.
  • Ability to prioritize tasks effectively and manage time efficiently in a dynamic environment.
Physical Requirements

Candidates must be able to lift and carry patients as needed, perform various physical tasks, and work safely with individuals of diverse sizes and medical conditions.

Compensation and Benefits

The annual base salary ranges from $74,672.00 to $112,112.00, complemented by a comprehensive benefits package including:
