Key Responsibilities- Provide direct patient care and support to residents with complex medical needs, including those with behavioral health conditions.
- Collaborate with our multidisciplinary team to develop and implement individualized care plans that promote optimal patient outcomes.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Administer medications and treatments as prescribed by physicians and other healthcare professionals.
- Monitor patient progress and adjust care plans as needed to ensure optimal patient outcomes.
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Illinois.
- Experience working in a skilled nursing facility or long-term care setting, preferably with a focus on behavioral health.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work flexible schedules, including weekends and holidays, as needed.
- Electronic medical records experience, preferably with Point Click Care (PCC) or similar systems.
