Special Service for Groups- AP Recovery is seeking a skilled Mental Health Case Manager to join our team. In this role, you will be responsible for planning, facilitating, implementing, and coordinating mental health and case management services to chronically mentally ill adults with a history of non-compliance with treatment, including substance abuse, homelessness, and/or incarceration.
Key Responsibilities:
- Provide mental health, case management, and substance use services, as well as linkages and referrals to ensure a continuous system of care.
- Assist with vocational, housing, self-help, and peer support services, including money management.
- Coordinate services with various public and private community health, mental health, and social service agencies in the treatment of clients.
- Engage in outreach and engagement activities.
- Report program activities, development, crises, and other pertinent information to the appropriate chain of command.
- Transport clients to connect with necessary community resources.
- Establish and maintain a therapeutic relationship with clients and family members.
- Participate in treatment recommendations and provide daily/weekly contact with clients to ensure a continuous system of care.
- Participate in cooperative arrangements and/or team conferences with interdisciplinary team members in the development and implementation of client treatment plans and aftercare plans.
- Maintain and submit billing records in a timely manner.
- Maintain and update client files and documentation in accordance with DMH guidelines.
- Maintain productivity rates as required.
Requirements:
- Bachelor's degree in Psychology, Social Work, or a related field, or a minimum of 2 years of experience working with the mentally ill population.
- Spanish speaking preferred.
- Verification of employment eligibility and background check.
- Reliable transportation, valid driver's license, and proof of automobile insurance coverage.
- Basic computer software programs, including Word processing, spreadsheet, and internet.
- Good writing and communication skills, strong interpersonal and social skills, and time management skills.
- Ability to work independently and as a member of a team.
