Position Overview:
In a collaborative environment, the Ambulatory Care Pharmacist delivers comprehensive pharmaceutical care services in accordance with established protocols. This role involves the management of patient medication therapies, patient education, and oversight of medication preparation and dispensing. The focus is on promoting high-quality, evidence-based, and cost-effective pharmaceutical care within designated clinical settings. Key Responsibilities:
  • In collaboration with the Department manager or designated personnel, advocates for rational and economical drug therapy for both inpatient and outpatient settings through active participation in the medication decision-making process alongside physicians and other healthcare professionals.
  • Provides mentorship and guidance to Pharmacist Residents, Interns, Pharmacy Technicians, and other support staff.
  • Conducts thorough medication history assessments based on referrals from healthcare providers.
  • Develops comprehensive treatment plans that may encompass drug therapy and lifestyle adjustments, ensuring smooth transitions across various care settings.
  • Identifies, addresses, and prevents medication-related issues, including untreated conditions, inappropriate drug choices, inadequate dosages, overdoses, and adverse drug reactions.
  • Implements, modifies, monitors, and recommends adjustments to drug therapies in accordance with established protocols, utilizing information gathered from patient interactions, objective monitoring data, and medical records.
  • Coordinates and interprets laboratory evaluations related to drug therapy, including monitoring serum drug concentrations and other relevant laboratory tests.
  • Engages in patient monitoring to assess and enhance outcomes, optimize drug utilization, ensure patient adherence, and minimize adverse drug reactions.
  • Educates patients regarding their medical conditions, including expected effects, potential side effects, the importance of adherence, and any specific instructions.
  • Ensures continuous improvement in the quality of pharmaceutical care services delivered.
  • Delivers educational sessions to healthcare providers and staff through various formats, including presentations, newsletters, and individual interactions.
  • Performs additional responsibilities as necessary.
