Job Title: Counselor/Social Worker - Partial Hospitalization Program

Brentwood Hospital is seeking a dynamic Counselor/Social Worker to join our Partial Hospitalization Program (PHP) team. As a member of our interdisciplinary team, you will provide quality case management and psychotherapeutic services to adult psychiatric patients, adult CD patients, adolescent patients, child patients, and their families.

Responsibilities:
  • Conduct psychosocial assessments to document history and identify preliminary issues for treatment focus.
  • Provide group and family process-oriented therapies, as well as conduct educational and other didactic groups for patients and family members.
  • Develop and coordinate individualized discharge plans for patients, utilizing treatment team and chart information to determine aftercare needs.
  • Identify and assess family or community resources to meet patients' aftercare needs.
Requirements:
  • Master's Degree in social work, counseling, or related field.
  • Experience in inpatient or outpatient mental health setting preferred.
  • LPC or LCSW license required.
