We are seeking a skilled Counselor/Social Worker to join our Partial Hospitalization Program (PHP) team at Brentwood Hospital. As a key member of our interdisciplinary team, you will provide high-quality case management and psychotherapeutic services to adult psychiatric patients, adult CD patients, adolescent patients, child patients, and their families.
Responsibilities- Conduct psychosocial assessments to identify preliminary issues for treatment focus and document patient history.
- Provide group and family process-oriented therapies, as well as educational and didactic groups for patients and family members.
- Develop and coordinate individualized discharge plans for patients, utilizing treatment team and chart information to determine aftercare needs.
- Identify and assess family or community resources to meet patients' aftercare needs.
- Master's Degree in social work, counseling, or a related field.
- Experience in inpatient or outpatient mental health settings preferred.
- LPC or LCSW license required.
