We are seeking a skilled Community Health Worker to join our team at Fairview Health Services. The successful candidate will be responsible for promoting health and wellness in our community by providing culturally sensitive support and connecting individuals with essential resources.

Key Responsibilities
  • Promote Health and Wellness: Provide health information and education to patients, families, and community members to promote healthy behaviors and lifestyles.
  • Connect Patients with Resources: Link patients to community resources, services, and support to ensure seamless care transitions and optimal health outcomes.
  • Support Care Management: Collaborate with healthcare providers, nurses, and care management teams to ensure comprehensive care planning and coordination.
  • Address Social Determinants of Health: Identify and address social determinants of health that impact patient outcomes, such as housing, food insecurity, and transportation.
  • Provide Social Support: Offer emotional support, counseling, and advocacy to patients and families to promote mental and emotional well-being.
Requirements
  • Experience: 3 years of experience working on community health projects, programs, or initiatives.
  • Certification: Community Health Worker Certificate required within 18 months of hire.
  • Education: College degree in health, human services, or related field preferred.
