- Connect Patients with Resources: Link patients to community resources, services, and support to ensure seamless care transitions and optimal health outcomes.
- Support Care Management: Collaborate with healthcare providers, nurses, and care management teams to ensure comprehensive care planning and coordination.
- Address Social Determinants of Health: Identify and address social determinants of health that impact patient outcomes, such as housing, food insecurity, and transportation.
- Provide Social Support: Offer emotional support, counseling, and advocacy to patients and families to promote mental and emotional well-being.
- Experience: 3 years of experience working on community health projects, programs, or initiatives.
- Certification: Community Health Worker Certificate required within 18 months of hire.
- Education: College degree in health, human services, or related field preferred.
