About the Role

We are seeking a highly organized and detail-oriented Intake Specialist to join our team at Alive Hospice. As a key member of our intake team, you will be responsible for responding to inquiries and intake calls, assisting with administrative duties, and providing exceptional customer service to our patients, families, and healthcare providers.

Key Responsibilities
  1. Represent Alive Hospice in a professional and courteous manner, promoting our services and collaborating with provider relations representatives.
  2. Provide backup support to the Director of Intake & Admissions as needed.
  3. Serve as a liaison and provide feedback from referral sources.
  4. Collaborate with physicians, social workers, discharge planners, and clinical coordinators to gather information for treatment plans.
  5. Answer and respond to multiple calls, including inquiry, intake, family, and healthcare provider calls.
  6. Provide accurate, consistent, and compassionate attention to each call.
  7. Maintain patient, family, and staff confidentiality.
  8. File, fax, log, and maintain data in an accurate and readily accessible fashion.
  9. Enter data, manipulate, and retrieve data in proper reporting structure.
  10. Route paperwork appropriately and in a timely manner.
  11. Assist with administrative duties as assigned by the Director of Intake & Admissions.
Requirements

To be successful in this role, you will need:

  • An Associate's Degree (A.A.) from a college or technical school.
  • One year's related experience and/or training.
  • Excellent communication and organizational skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Proficiency in computer and telephone skills.
