Case Manager

At Transitional Center Inc., we are seeking a skilled Case Manager to join our team. As a Case Manager, you will act as the single point of contact for all parties involved in a youth's well-being and all aspects of care. Your primary responsibility will be to ensure proper utilization of services and resources, and provide assistance within, between, and outside of the facility.
Key Responsibilities:
- Supervise assigned youth in the treatment milieu
- Provide therapeutic interactions and support to youth and their families
- Ensure proper utilization of services and resources
- Provide assistance within, between, and outside of the facility
Requirements:
- Bachelor's Degree
- At least 25 years of age
- Two years of full-time experience in a residential child care program
- Ability to work cooperatively with administration, staff, and persons internal and external to the program
- Valid driver's license and current/valid insurance for vehicle
Working Conditions:
- Mental/visual stress related to viewing of computer screens and printed material for prolonged periods
- Lifting limited to a box of forms weighing 7-8 pounds
- Prolonged periods of sitting
Physical Elements Required:
- Lifting and carrying 5-7 pounds
- Frequent bending, stooping kneeling, and reaching, stretching
- Standing for long periods of time
Mental Performing Elements:
- Ability to maintain a calm demeanor
- Ability to work within a closed environment
- Ability to focus and maintain decorum
Competency Requirements:
- Competent in all aspects of technology including PC, IPAD, and iPhone
- Ability to quickly learn DCFS Software and all other agency software
Performance Areas:
- Employee performance evaluations will be conducted annually
- Employee will be evaluated on four measures: E=Excellent, S=Satisfactory, N=Needs Improvement, and U=Unsatisfactory
Termination Policy:
- Employee with 3 or more unsatisfactory areas will be terminated, transferred to another position, or placed on a professional development plan
