Position Overview

The Ambulatory Care Clinical Pharmacist collaborates with a multidisciplinary team to deliver comprehensive management of disease states and oversee clinical care for a designated patient population. This role is pivotal in enhancing coordinated care across the health system, aiming to improve quality and safety outcomes.

The Clinical Pharmacist is tasked with educating healthcare professionals on pharmacotherapy management and acts as a key resource for medication-related inquiries, while also fostering collaboration between pharmacy and other healthcare disciplines.

Key Responsibilities

The Clinical Pharmacist will:

1. **Conduct Disease State Management**: Operate under a collaborative practice agreement for patients referred to the pharmacist-led program within the ambulatory care clinics.

2. **Information Collection and Analysis**: Ensure thorough collection of subjective and objective patient data, analyzing it to understand the patient's medical and medication history.

3. **Assessment and Problem Formulation**: Assess gathered information to create a medication therapy problem list, prioritizing recommendations to enhance medication use and achieve clinical objectives.

4. **Care Plan Development and Implementation**: Collaborate with the healthcare team and patients to develop and execute a care plan that considers co-morbidities, cost-effectiveness, adherence barriers, and evidence-based recommendations.

5. **Patient and Family Education**: Provide direct education to patients and families to ensure understanding and agreement with therapy plans.

6. **Follow-Up and Monitoring**: Offer ongoing follow-up to optimize care plans and resolve medication therapy issues, including utilizing remote monitoring tools as necessary.

7. **Education and Training**: Deliver periodic education to clinic and system providers on updates to guidelines and formularies, and provide educational experiences for pharmacy students and residents as needed.

Minimum Qualifications

- PharmD degree from an accredited pharmacy school.
- A minimum of two years of relevant clinical pharmacy experience or formal residency training in a clinic-based, ambulatory setting. A PGY2 in Ambulatory Care may substitute for two years of experience.
- Must possess or be eligible for pharmacy licensure in North Carolina within 30 days of hire.
- American Heart Association Basic Life Support (BLS) certification is required.
- Commitment to obtaining continuing education credits as mandated by the North Carolina Board of Pharmacy.
