Key Responsibilities- Conduct thorough patient assessments, analyze data, and develop personalized care plans to optimize patient outcomes.
- Administer medications in a safe and timely manner, adhering to established protocols and guidelines.
- Collaborate with interdisciplinary teams to coordinate patient care, ensuring seamless transitions and effective communication.
- Monitor patient conditions, identify potential issues, and take proactive steps to prevent complications.
- Provide education and support to patients and their families, empowering them to take an active role in their healthcare.
- Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD degree.
- Current licensure as a pharmacist in the state(s) where practicing.
- 2+ years of recent experience in an acute care setting, with a focus on patient care and medication management.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
