Job Description

As a Primary Care Provider - PRN/Part Time Home Risk Assessment Specialist, you will conduct comprehensive health assessments in various settings, including homes, skilled facilities, and telehealth visits. Your expertise will help identify chronic conditions, provide personalized education, and facilitate collaboration with healthcare teams to improve patient outcomes.

Key Responsibilities
  • Conduct adult and geriatric assessments, medical history reviews, diagnosis, treatment, health education, and referrals as needed.
  • Collaborate with physicians, case managers, social workers, family members, caregivers, and other healthcare professionals to ensure comprehensive care.
  • Work closely with Primary Care Physicians (PCPs) on patient education and follow-up, promoting seamless care coordination.
  • Provide services in a variety of venues, adapting to the needs of each patient and environment.
Required Skills and Qualifications

To succeed in this role, you must possess:

  • A Master's Degree in Nursing or equivalent experience with satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain it.
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • A minimum of 1 year of experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with Employee Health/Medical Record software.
  • Excellent verbal and written communication skills.
  • The ability to travel, hold a valid driver's license, and maintain adequate automobile insurance.
