As a Nurse Practitioner or Physician Assistant, you'll play a vital role in our HouseCalls product, conducting annual clinical assessments and providing preventive care to patients across the country.

Key Responsibilities:

  • Conduct in-home assessments, including medical history reviews, medication reconciliation, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing
  • Identify diagnoses and develop care management plans
  • Communicate findings with patients' primary care physicians
  • Educate patients on disease processes, medication, and compliance

Requirements:

  • Active NP or PA license, with ability to obtain national certification
  • Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions
  • Reliable transportation for home visits

Preferred Qualifications:

  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Home health care or home visit experience
  • Ability to travel up to 5% within licensed states
