Job Summary

We are seeking a Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time Home Risk Assessment provider. As a member of our team, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, as well as provide health education and collaborate with primary care physicians.

Responsibilities
  • Conduct in-home assessments and medical history reviews
  • Diagnose and treat patients as needed
  • Provide health education and counseling
  • Collaborate with primary care physicians and other healthcare professionals
  • Document patient information and treatment plans
Requirements
  • Master's degree in nursing or physician assistant studies
  • Current RN and NP or PA licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • 1 year of experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
