Job Summary:

The Care Manager will be responsible for providing exceptional care and support to our clients, while also promoting our services and building relationships with referral sources and community partners.

Key Responsibilities:
  • Conduct in-home assessments with seniors and their families to identify their care needs and develop personalized care plans.
  • Provide ongoing care management and communication with clients to ensure their needs are met and they are satisfied with our services.
  • Represent our company at community events and build relationships with referral sources and partners.
  • Collaborate with our staffing team to ensure clients' needs are met and concerns are addressed.
  • Support client retention through hands-on attention, client appreciation initiatives, and relationships with clients and their families.
  • Provide after-hours support for clients, referral partners, and prospective clients, including evenings and weekends.
Requirements:
  • Degree in social work, human services, gerontology, counseling, communication, or a related field from an accredited university.
  • Three or more years of experience in case management and/or social advocacy with older adults.
  • Ability to pass a DOJ/FBI fingerprint background check.
  • Ability to work some nights and weekends.
