Diabetes Care Coordinator

The Nurse (Diabetes) - ESEP/MP will be responsible for managing care of clients diagnosed with or at risk for Type 2 Diabetes Mellitus; utilizing professional judgment and the IHS Diabetes Standards of Care. They will also teach the pathophysiology of diabetes, diet/exercise, medications, prevention and management of acute and chronic complications, self-care and management of diabetes.
