Job Title: Licensed Practical Nurse

Aveanna Healthcare LLC is seeking a skilled Licensed Practical Nurse (LPN/LVN) to join our team. As a LPN/LVN, you will be responsible for providing high-quality patient care in a home health setting.

Job Summary:

The LPN/LVN will work under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. This includes assessing patient needs, developing and implementing care plans, and providing education to patients and their families.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse
  • Assess patient needs and develop and implement care plans
  • Provide education to patients and their families on disease management and self-care techniques
  • Document all patient interactions and care provided in a timely and accurate manner
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
Requirements:
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
Physical Requirements:
  • Must be able to speak, write, read, and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must be able to sufficiently reposition patients and move equipment without assistance
