Job Title: Nurse Care Manager
Job Summary: STGi is seeking a skilled Nurse Care Manager to provide high-quality patient care in our Community Based Outpatient Clinic. As a key member of our healthcare team, you will be responsible for coordinating patient care, managing medical records, and ensuring seamless communication between healthcare providers.
Responsibilities:
- Provide professional nursing care to patients, including assessing their medical needs, developing care plans, and implementing treatment protocols.
- Collaborate with healthcare providers to ensure comprehensive patient care, including ordering lab tests, prescribing medications, and referring patients to specialists.
- Manage patient medical records, including documenting patient information, updating medical histories, and maintaining confidentiality.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Current RN licensure in the state of practice.
- Minimum 3 years of experience in medical-surgical nursing, outpatient care, or primary care.
- Knowledge of nursing principles, procedures, and techniques.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
