Job Title: RN Home Care Liaison

Home Helpers of Alpharetta is seeking a compassionate and driven Registered Nurse (RN) to join our management team as a Home Care Liaison. This role plays a pivotal part in ensuring patients receive the highest quality care while serving as the bridge between healthcare providers, patients, and families.

Key Responsibilities:
  • Conduct initial and supervisory assessments of clients in their homes
  • Develop and implement home care plans for clients
  • Monitor client progress and make necessary adjustments to care plans
  • Coordinate with other healthcare professionals to ensure clients receive the best possible care
  • Community outreach and education to develop home care referrals
  • Maintain accurate and up-to-date records of client care plans and progress
  • Provide support and guidance to clients, family members, and caregivers
  • Ensure compliance with all applicable regulations and laws
Requirements:
  • Active RN license in Georgia
  • Minimum of 2 years of home care experience, with at least 1 year in a supervisory role
  • Strong clinical skills and knowledge of home care practices
  • Strong organizational, communication, and interpersonal skills
  • Ability to work independently and in a collaborative team environment
  • Compassionate and empathetic attitude toward clients and their families
  • Reliable transportation and valid driver's license
  • Clear background check and drug screening
