Care Coordinator

We are seeking a skilled Care Coordinator to join our team at Chapel Haven Schleifer Center Inc. This role involves providing support and instruction in daily living skills to individuals with learning disabilities or developmental disabilities.
Key Responsibilities:
- Teaching and modeling meal planning, grocery shopping, and meal preparation skills
- Assisting with personal hygiene and grooming
- Providing guidance on social relationships and problem-solving
- Completing daily progress notes and medication administration as required
Requirements:
- Bachelor's Degree in Human Services or related field
- 4+ years of experience working with individuals with disabilities
- State of Connecticut Medication Administration Certification (as required)
- Valid CT driver's license
Salary and Benefits:
The estimated salary for this role is $18.75 per hour, based on the location in New Haven, CT. We offer a full benefits package, including paid time off and tuition reimbursement.
