Position Overview

The Clinical Social Services Coordinator plays a vital role in enhancing the well-being of participants and their families under the guidance of the Center Administrator. This position involves the strategic planning, organization, and execution of social work services tailored to meet the unique needs of PACE participants.


Key Responsibilities:
  • Conduct comprehensive assessments and provide treatment, education, and counseling to participants and their caregivers.
  • Engage in individual participant interactions and maintain appropriate collateral communications.
  • Facilitate participant and family education, ensuring they are well-informed and supported.
  • Offer resources and address mental health concerns as they arise.
  • Manage ongoing cases, advocating for the needs of participants and caregivers.
  • Oversee disenrollment processes when necessary.

The Clinical Social Services Coordinator serves as a crucial link between the interdisciplinary team, caregiver representatives, and community organizations.


Requirements:

Applicants must possess medical clearance for communicable diseases and current immunizations prior to engaging with participants. A valid driver's license, proof of insurance, and reliable transportation are essential, in accordance with EveryAge's auto insurance standards.


Educational Qualifications:

A Master's degree in Social Work from an accredited institution is required. Membership in the Academy of Certified Social Workers (ACSW) or other NASW-recognized certifications is preferred. Additionally, CPR and First Aid certifications are mandatory.


Experience:

A minimum of two years' experience in a collaborative interdisciplinary environment, such as a hospital, nursing home, or community-based setting, is preferred. At least one year of experience working with frail or elderly populations is also required.
