RN Supervisor for PDN Services

Summit Home Care, a reputable and family-owned home care agency, is seeking an experienced and compassionate RN Supervisor to join our team of dedicated professionals. As an RN Supervisor, you will play a vital role in ensuring the delivery of high-quality care to our clients.
Key Responsibilities:- Collaborate with the Patient Coordinator to ensure accurate clinical documentation for case openings.
- Develop and revise the plan of care, medical orders, Medication Administration Records (MAR), and Treatment Authorization Requests (TAR) as needed.
- Conduct regular client visits, both in-home and virtually, to assess client needs and provide support.
- Provide in-home supervision every 60 days or as necessary to ensure the quality of care.
- Participate in problem-solving processes to address EMR computer education needs and achieve established outcomes for users.
- Act as a liaison between Summit and the community, clients, facilities, and other stakeholders.
- Evaluate the outcome of care and provide support to ensure that services meet the client's needs.
- Communicate changes in client needs to physicians and other staff members.
- Current RN license in the state.
- 2+ years of supervisory experience.
- Previous experience in Home Care.
