Licensed Practical Nurse/LPN

The Licensed Practical/Vocational Nurse (LVN/LPN) plays a vital role in providing nursing care and teaching to clients and families. As an Assistant to the physician and Registered Professional Nurse, this individual is supervised by the RN and works collaboratively to deliver high-quality patient care.
Responsibilities- Participate in the planning and coordination of total client care in conjunction with the RN, agency policies, and the physician's Plan of Care.
- Follow the nursing care plan for each assigned client, ensuring accurate and timely implementation.
- Observe and evaluate the client's condition, applying knowledge of nursing skills to provide optimal care.
- Accurately report and record the client's condition and care, including signs and symptoms that may indicate a change.
- Prepare clinical/progress notes on the day of the visit and incorporate them into the clinical record weekly, summarizing key findings in cooperation with the RN supervisor.
- Assist clients with activities of daily living while teaching appropriate self-care techniques.
- Provide and maintain a safe environment for the client, ensuring a secure and comfortable space.
- Collaborate with the physician and Registered Nurse in complex nursing situations and perform specialized procedures as needed.
- Prepare equipment and materials for treatments, adhering to aseptic technique as required.
- Participate in case conferences, inservices, and other programs and meetings as needed.
- Perform duties within the limits of preparation and experience, ensuring a high level of professionalism.
- Confirm, on a weekly basis, the scheduling of visits with the nurse to coordinate necessary visits with other personnel.
- Perform treatments as assigned per physician's plan of care, adhering to established protocols and guidelines.
- Reports to the Director of Nursing Services.
- Must have a car with required insurance coverage and a state driver's license or reliable transportation.
- Able to communicate verbally and in writing to the extent required by the position.
- Able to physically perform the duties required by the position.
- Able to travel to prospective clients' residences.
