Your Responsibilities

As a Clinical Nurse Liaison, you will report to the AVP of Business Development and be an integral part of our exceptional team. Your responsibilities will include:

  • Ensuring efficient and timely intake and referral processes to meet the needs of patients, families, and referral sources.
  • Completing the admissions process for referred patients in urgent situations, as staffing allows, and for inpatient hospice admissions.
  • Providing accurate and timely information about hospice services to all inquiries.
  • Responding promptly to calls from referral sources and initiating the necessary paperwork for the intake/referral process.
  • Advocating for patients by evaluating and developing care plans and obtaining informed consent for hospice services.
  • Engaging with various account types, including Skilled Nursing Facilities, Assisted Living Facilities, Hospitals, and Physicians.
  • Conducting educational sessions on the benefits of hospice care.

Qualifications for Success

To thrive as a Clinical Nurse Liaison, you should possess:

  • A compassionate heart for hospice care, demonstrating kindness and empathy towards clients, their patients, and families.
  • A valid RN license in the state(s) of employment.
  • A minimum of 2 years of healthcare experience, preferably in hospice, home health, clinical liaison roles, or similar positions.
  • A strong reputation and respect within your community.
