Job Summary

We are seeking a skilled and compassionate Professional Hospice Case Manager to join our team at Arkansas Hospice, Inc. This is a rewarding opportunity to provide high-quality patient care and support to individuals and their families in need.

Responsibilities
  • Manage patient care functions after initial admission and assignment to a designated case manager.
  • Assist in the admission process as requested.
  • Interpret and communicate policies and procedures to nursing staff and others to ensure understanding and compliance.
  • Coordinate and collaborate with interdisciplinary team members to manage the plan of care and meet patient/family needs.
  • Update the Plan of Care on an ongoing basis and keep other team members informed about patient progress and changing care needs.
  • Facilitate effective communication with patients/families and healthcare providers.
  • Maintain communication with attending physicians and community organizations regarding resources for assigned patients/families.
  • Monitor pain and symptom control to ensure optimal patient comfort.
  • Coordinate CNA scheduling needs with Patient Care Coordinator/Area Manager or designee.
  • Initiate referrals for hospice-related care and services.
  • Serve as a preceptor for nurses in orientation, students, and trainees.
  • Review documentation of staff and ensure it is complete, accurate, and timely according to organizational policies and practice standards.
  • Participate in interdisciplinary team meetings, case conferences, care planning activities, and committee meetings as assigned.
Requirements
  • Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of one year nursing experience in a hospital, home health agency, hospice, or related program.
  • Licensed as a Registered Nurse (RN) in Arkansas.
  • Valid Arkansas driver's license, personal automobile, automobile insurance, and personal cell phone required.

  • Knowledge of palliative care with emphasis on symptom control associated with terminal illness.
  • Holistic approach toward patient care and the nursing process as it applies to hospice patients and families.
  • Awareness and acceptance of hospice philosophy and principles of care.
  • Ability to organize and prioritize workloads to meet deadlines.
  • Effective communication and teamwork skills.
  • Basic safety policies and procedures awareness.
  • Good judgment and ability to maintain confidentiality of information.
  • Work in a fast-paced environment.
Benefits

This position offers a competitive salary of $63,000 annually, along with opportunities for professional growth and development in a supportive and collaborative work environment.

The estimated annual salary range for this role is $60,000 - $70,000 based on location and qualifications.
