Job Title: Social Worker - Psychosocial Services

Parkland Health & Hospital System is seeking a skilled Social Worker to join our team. As a Social Worker - Psychosocial Services, you will play a vital role in ensuring the health and well-being of our patients and their families.

Job Summary:

We are looking for a compassionate and detail-oriented Social Worker to perform psychosocial screenings and assessments, develop and implement psychosocial plans of care, and provide counseling and support to patients and their families.

Responsibilities:
  1. Perform psychosocial screenings and assessments to identify patient needs and develop appropriate interventions.
  2. Counsel patients and families regarding emotional, social, and financial consequences of illness and/or disability.
  3. Participate in discharge planning activities to ensure timely patient discharge and promote optimal utilization of patient and community resources.
  4. Communicate daily with the interdisciplinary team regarding status of discharge planning activities.
  5. Provide intervention in cases involving child or adult abuse/neglect, domestic violence, guardianship, foster care, adoption, mental health level II placement, and sexual assault.
  6. Document screening, assessment, interventions, and outcomes to ensure effective communication.
  7. Manage referrals and communications from various sources to ensure optimal customer service.
  8. Maintain knowledge of rules, regulations, policies, procedures, laws, and guidelines that impact the Social Work field.
  9. Report assigned data as needed to ensure timeliness and accuracy.
  10. Identify ways to improve work processes and improve customer satisfaction.
  11. Stay abreast of the latest developments, advancements, and trends in the field of Social Work.
Requirements:
  • Master's degree in Social Work.
  • Licensed by the Texas State Board of Social Work.
  • Current Heartsaver CPR course completion card.
  • Satori Alternatives for Managing Aggression (SAMA) certification, level 1-4.
  • Excellent interpersonal and counseling skills.
  • Ability to communicate effectively with a diverse population of patients, medical and nursing staff, and community agencies.
