Job Summary

The Department of Physical Medicine & Rehabilitation at UT Southwestern Medical Center is seeking a skilled Social Worker to join their team. As a Social Worker, you will play a vital role in providing counseling services, connecting patients with community resources, and facilitating their transition to post-acute care.

Key Responsibilities
  • Provide individual and group counseling services to patients, addressing their psychosocial needs and concerns.
  • Assess patients' needs and connect them with community resources, such as meal and transportation services, job-related resources, and assistance with adjustment to disability.
  • Collaborate with the healthcare team to develop and implement patient care plans, ensuring a smooth transition to post-acute care.
  • Document patient interactions, progress, and outcomes, maintaining accurate and up-to-date records.
  • Participate in multidisciplinary rounds, contributing to the development of patient care plans and ensuring effective communication among team members.
Requirements
  • Master's degree in Social Work from a Council of Social Work Education-accredited institution.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas.
  • Temporary licensure acceptable during the first 90 days of employment.
  • 2 years of experience in a healthcare setting, preferably in social work or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
