Senior Case Manager

N Street Village is seeking a dedicated Senior Case Manager to provide essential services to our clients at Sharon's Place, a nine-month transitional housing program for women experiencing homelessness.
Key Responsibilities:
- Develop and maintain a safe and supportive environment for clients.
- Coordinate referrals and solutions for housing, financial, employment, recovery, and mental health challenges.
- Co-facilitate client groups and provide bi-weekly face-to-face case management services.
- Maintain regular communication with the program manager about client successes and challenges.
- Collaborate with partner agencies and professionals to coordinate client needs.
- Participate in organization-wide meetings and committees, departmental meetings, team meetings, and training as assigned.
Qualifications:
- A combination of education and experience in mental health, physical health, addiction, aging, or trauma.
- High school diploma or Bachelor's Degree in a related social service discipline, with 2 years of experience providing case management services.
- Knowledge of DC social services resources and compliance requirements.
