Case Management Director

We are seeking a highly skilled and experienced Case Management Director to lead our case management team and oversee the department's daily activities. The ideal candidate will have a strong background in case management, leadership, and patient care coordination.
Key Responsibilities- Provide leadership and direction to the case management team
- Oversee the department's daily activities and ensure efficient and effective patient-centered care
- Coordinate case management and utilization functions to achieve identified objectives
- Assign duties to staff members based on ratios, volumes, skills, and departmental needs
- Identify and mitigate problems as they occur and address performance issues
- Develop and recommend department operating budgets and ensure the department operates within budget
- Manage daily operations, including staffing levels and assignments
- Participate in meetings, serve on committees, and represent the department and hospital in community outreach efforts
- Establish and promote relationships with clinical team members, physician advisors, and individual physicians
- Contribute to the development of physician strategy as related to documentation and resource management
- Escalate issues to the department director as needed
- Bachelor's degree in Nursing (BSN) required
- 3-4 years of relevant acute care work experience required
- 2-3 years of case management experience preferred
- 2-3 years of progressive leadership experience preferred
- Registered Nurse (RN) licensure and/or Compact State Licensure in the District of Columbia or Maryland required
- Certified Case Manager (CCM) certification preferred
- Effective verbal and written communication skills
- Excellent interpersonal and customer service skills, especially serving geriatric patients
- Strong analytical and critical thinking skills
- Strong community engagement and facilitation skills
- Advanced project management skills
- Cultural competency
- Proficient computer skills, including Microsoft applications and electronic medical records
