Position Overview: Join Tribute Home Care as a Client Care Coordinator and contribute to redefining the standards of senior care.

Your Impact: In this pivotal role, you will engage with prospective and existing clients, their families, and other care professionals to ensure a seamless and positive experience from the initiation of services.

Key Responsibilities:

  • Deliver a warm and informative introduction to Tribute by promptly addressing inquiries and conducting in-person assessments.
  • Oversee the establishment of care services by developing personalized client care plans, effectively matching Caregiver Associates with clients, and orienting caregivers to the specific needs of each client.
  • Maintain proactive communication with all stakeholders involved in client care.
  • Regularly review and adjust care plans as necessary to meet evolving client needs.
  • Foster strong, trusting relationships with clients and their families.

Community Engagement: In addition to client relationships, you will cultivate connections with community referral sources. We seek a candidate who is innovative, resourceful, and enthusiastic about promoting and enhancing a service they can take pride in.

Travel Requirements: This role necessitates regular travel to assess and visit clients in various regions.

Work Environment: Enjoy the flexibility of working from home when not engaging with clients or attending meetings.

Qualifications:

  • Bachelor's degree from an accredited institution.
  • Experience in a clinical environment.
  • Prior experience in managing caseloads.
  • A strong commitment to enhancing customer experience and service excellence.
  • Excellent writing and communication skills.
  • Meticulous attention to detail and organizational skills.
  • A collaborative mindset.
  • Ability to travel regularly to meet clients.
