Job Title: Social Worker LCSW Hospice

Job Summary:

We are seeking a skilled Social Worker LCSW to join our Hospice team at PruittHealth. As a Social Worker LCSW, you will provide professional counseling, assessment, and support services to patients and their families, helping them navigate the complexities of terminal illness and end-of-life care.

Key Responsibilities:

  • Provide psychosocial assessments and counseling to patients and families, identifying and addressing their unique needs and concerns.
  • Develop and implement individualized care plans, collaborating with the interdisciplinary team to ensure comprehensive care.
  • Provide crisis intervention and suicide assessment, as needed.
  • Participate in the development and implementation of the plan of care, collaborating with other team members to ensure seamless care.
  • Provide bereavement services and support to families, referring them to additional resources as needed.
  • Recruit, train, and supervise hospice and home care volunteers, ensuring they are equipped to provide high-quality support services.
  • Assist with discharge planning, ensuring a smooth transition for patients and families.
  • Participate in ongoing inservice education, staying up-to-date on best practices and industry developments.

Requirements:

  • Masters Degree in Social Work (MSW) required.
  • One year experience preferred.
  • Masters level social workers must be eligible for state licensure/certification and pass the licensure exam within six months of employment.
  • Current Drivers License and Automobile Liability Insurance.

Preferred Qualifications:

  • Orientation and training to complete assigned duties effectively.
  • Ability to work a flexible schedule.
  • Positive criminal history check, Office of the Inspector General Sanction Check, and licensure validations.
