Job Summary

Aveanna Healthcare is seeking a skilled Licensed Practical Nurse (LPN) to join our team in Yakima, WA. As an LPN, you will play a vital role in delivering high-quality patient care and collaborating with our interdisciplinary team to achieve exceptional patient outcomes.

Key Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to address these needs.
  • Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
  • Document patient progress, including medical history, treatment plans, and outcomes, in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of an accredited school of nursing with a current, unrestricted LPN license in the state of practice.
  • Current CPR certification and ability to lift 50 pounds.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to travel to patient homes and work independently with minimal supervision.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience and knowledge of relevant regulations and guidelines.
