Job Title: Lead Primary Care Provider
Key Responsibilities:
- Conduct comprehensive evaluations and provide treatment to patients at the healthcare facility in accordance with established medical standards.
- Assess the quality of patient care by utilizing available data and performing thorough chart reviews.
- Facilitate the coordination of patient services, which includes specialty referrals, hospital collaboration, and home health services.
- Engage actively in care team meetings, offering medical insights and expertise.
- Promote a collaborative environment within the practice by exhibiting professionalism and approachability.
- Ensure prompt and precise documentation of medical records, working closely with quality-focused coding specialists.
- Comply with policies and protocols set forth by Clinical Leadership.
- Participate in discussions with the Regional Medical Director regarding care quality, outcome metrics, policies, procedures, and record management.
- Identify and explore opportunities for the expansion of new or existing services within the healthcare center.
- Partake in the local primary care on-call program as required.
- Maintain adherence to the standards set by licensing, certification, and accrediting organizations.
- Devote 100% of your professional time to direct patient care, encompassing patient interactions and administrative responsibilities related to patient care.
Qualifications and Skills:
- Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatrics.
- Possession of an active DEA license.
- Graduated from an accredited MD or DO program.
- Holds a current and unrestricted medical license in the state of practice.
- Exhibits strong interpersonal skills for effective communication with both colleagues and patients.
- Fully dedicated to the integrated team-based care model.
- Demonstrates flexibility and adaptability to work within a value-based care framework.
- Medicare Provider Number (Preferred).
- Medicaid Provider Number (Preferred).
- 2-5 years of relevant professional experience (Preferred).
- Experience in managing a Medicare Advantage patient panel (Preferred).
- Familiarity with Medicare guidelines and coverage (Preferred).
- Bilingual proficiency is advantageous (Preferred).
- Knowledge of HEDIS quality indicators (Preferred).
