About the Role

Evolve Bradford Health Services is seeking an experienced Intake Coordinator to play a crucial role in the admission process. As the single point of entry, this individual will ensure seamless transitions for patients and their families.

Key Responsibilities
  • Conduct thorough pre-admission screenings and schedule assessments in a timely manner.
  • Collaborate with various departments, including business office, nursing, and clinical staff, to facilitate timely patient admissions.
  • Refer inquiries to relevant agencies and community resources when facility assistance or admission is not suitable.
  • Provide information and referral services to both internal and external customers.
  • Interview patients, family members, and/or collateral sources to ensure intake criteria are met, reviewing prospective admissions against approved criteria, policies, and procedures.
  • Present clinical information to a member of the medical staff for approval of intake.
  • Offer crisis intervention services as needed.
  • Verify insurance coverage upon intake and ensure accurate documentation of all admissions processes.
  • Maintain ongoing communication with referral sources regarding the status of referred patients and provide education about available services.
  • Provide clerical support to admissions processes, including collating admission folders, auditing patient charts, and monitoring patients in internal waiting areas.
Requirements
  • High school diploma or equivalent required; Bachelor's degree in social work, psychology, counseling, or related field preferred.
  • Minimum of one year of experience in healthcare admissions, preferably in mental health or substance abuse settings.
  • Experience in patient assessment, referral, treatment planning, and communication with external review organizations or similar entities.
