Role Overview: The Healthcare Coordinator plays a crucial role in assisting patients with complex health conditions to navigate the healthcare landscape. This position focuses on empowering patients by serving as an educator, resource, and advocate for them and their families, ensuring they achieve the highest quality of life. The Healthcare Coordinator collaborates with multidisciplinary teams, including physicians, nurses, pharmacists, and social workers, to facilitate effective transitions of care. This role operates in a flexible, self-directed environment, performing all nursing responsibilities within the scope of a LVN/LPN license as mandated by the state board of nursing.

Key Responsibilities:

  • Collaborate with providers and clinic personnel to identify high-risk patients during care transitions, aiming to reduce emergency room visits and hospital admissions.
  • Support ongoing care for patients with chronic conditions by conducting health assessments, initiating medication reconciliation, and providing education on self-management.
  • Facilitate the transition of patients from inpatient to outpatient care by assessing transitional needs, coordinating medication reconciliation, and ensuring follow-up with primary care providers.
  • Document patient interactions accurately and promptly in the electronic medical record system.
  • Engage in daily huddles and Patient Care Coordination meetings to enhance team collaboration.
  • Prepare and present reports for weekly meetings as required.
  • Maintain up-to-date knowledge in nursing practices and evidence-based methods.
  • Perform clinical tasks within the designated scope of practice.
  • Execute additional related duties as assigned.

Required Qualifications:

  • Active and unrestricted Licensed Practical/Vocational Nurse license in the state of employment.
  • Current Basic Life Support (BLS) certification.
  • A minimum of 2 years of experience in a clinical, hospital, or physician's office setting.
  • Demonstrated knowledge of chronic illnesses, particularly COPD/asthma, diabetes, CHF, and IHD.
  • Proficient in using electronic medical records.
  • Strong verbal and written communication skills.
  • Excellent interpersonal abilities.
  • Proven capacity to work effectively with diverse teams.
  • Exceptional organizational and prioritization skills.

Preferred Qualifications:

  • IV Certification.
  • Experience in patient education and motivational interviewing.
  • Background in outbound patient communications.
  • Fluency in both English and Spanish, written and verbal.
