Role Overview: The Healthcare Coordinator plays a crucial role in assisting patients with complex health conditions to navigate the healthcare landscape. This position is focused on empowering patients by serving as an educator, resource, and advocate, ensuring they achieve the highest quality of life. The Healthcare Coordinator collaborates with multidisciplinary teams, including physicians, nurses, pharmacists, and social workers, to facilitate effective transitions of care. This role requires a self-directed approach, performing nursing duties within the scope of a LVN/LPN license.
Key Responsibilities:
- Collaborate with providers and clinic staff to identify high-risk patients during transitions of care, aiming to reduce emergency room visits and hospital readmissions.
- Support ongoing care for patients with chronic conditions by conducting health assessments, initiating medication reconciliation, and providing patient education.
- Facilitate the transition of patients from inpatient to outpatient settings by assessing transitional needs and ensuring follow-up care.
- Coordinate with healthcare providers to establish or update care plans.
- Document patient interactions accurately and promptly in the electronic medical record.
- Participate in daily team meetings and care coordination discussions.
- Prepare reports for weekly meetings as required.
- Maintain professional competence and stay updated on evidence-based practices.
- Perform other related duties as assigned.
In this role, you will be recognized for your contributions in an environment that encourages growth and provides clear guidance for success.
Required Qualifications:
- Active and unrestricted LVN/LPN license in the state of employment.
- Current BLS certification.
- Minimum of 2 years of experience in a clinical or hospital environment.
- Strong knowledge of chronic diseases, including COPD, diabetes, and heart failure.
- Proficient in using electronic medical records.
- Excellent verbal and written communication skills.
- Strong interpersonal skills and ability to work effectively within multidisciplinary teams.
- Exceptional organizational and prioritization abilities.
Preferred Qualifications:
- IV Certification.
- Experience in patient education and motivational interviewing.
- Fluency in both English and Spanish.
