Role Overview: The Healthcare Coordinator plays a crucial role in aiding patients with high-risk health conditions in navigating the healthcare landscape. This position is dedicated to fostering patient empowerment by serving as an educator, resource, and advocate for patients and their families, ensuring optimal quality of life.

Key Responsibilities:

  • Collaborate with healthcare providers and clinic personnel to identify high-risk patients during care transitions, aiming to minimize emergency room visits, hospital admissions, and readmissions.
  • Facilitate ongoing care for patients with chronic conditions by conducting health assessments, initiating medication reconciliation, and providing education on self-management.
  • Assist in the transition of patients from inpatient to outpatient settings by assessing transitional needs, establishing contingency plans, and ensuring follow-up care with primary care providers.
  • Coordinate with healthcare providers to create or update care plans.
  • Maintain accurate and timely documentation in electronic medical records.
  • Engage in daily huddles and Patient Care Coordination meetings.
  • Prepare and present reports for weekly meetings as required.
  • Stay updated on nursing practices and evidence-based methods.
  • Perform clinical tasks within the scope of practice.
  • Execute additional related duties as assigned.

Required Qualifications:

  • Active and unrestricted Licensed Practical/Vocational Nurse license in the state of employment.
  • Current Basic Life Support (BLS) certification.
  • Minimum of 2 years of experience in a clinical or hospital environment.
  • Solid understanding of chronic diseases, particularly COPD/asthma, diabetes, CHF, and IHD.
  • Proficient in using electronic medical records.
  • Excellent verbal and written communication skills.
  • Strong interpersonal abilities.
  • Ability to work effectively with multidisciplinary teams.
  • Exceptional organizational and prioritization skills.

Preferred Qualifications:

  • IV Certification.
  • Experience in patient education and motivational interviewing.
  • Familiarity with outbound patient communications.
  • Fluency in both English and Spanish.
