Job Summary

We are seeking a highly skilled and compassionate Oncology Nurse Navigator to join our team at Stony Brook Medicine. As a key member of our cancer care team, you will play a vital role in ensuring that patients receive seamless and high-quality care throughout their cancer journey.

Key Responsibilities
  • Provide personalized navigation and support to patients and their families, addressing their unique needs and concerns.
  • Collaborate with healthcare providers, social workers, and other members of the cancer care team to ensure timely and effective transitions of care.
  • Develop and implement patient-centered care plans, taking into account the patient's medical, emotional, and social needs.
  • Facilitate access to resources and services, including clinical trials, support groups, and community programs.
  • Provide education and guidance to patients and their families on cancer treatment options, side effects, and survivorship care.
  • Participate in quality improvement initiatives and contribute to the development of best practices in cancer care.
Requirements
  • Current RN licensure in New York State.
  • Minimum 5 years of acute nursing experience, with at least 3 years in oncology.
  • Bachelor's degree in nursing.
  • Preferred: Master's degree in nursing, Certified Oncology Nurse (CON) certification, and experience in case management, quality management, and/or patient education.
