We are seeking a skilled Nurse Practitioner to join our team at Three Rivers Home Health Services.
About the Role:
- Job Summary: As a Registered Nurse (RN) in Home Health, you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
Key Responsibilities:
- Coordinate Total Plan of Care: Work with other healthcare professionals to achieve and maintain continuity of patient care by planning and exchanging information with the physician, agency personnel, patient, family, and community resources.
- Develop and Implement Care Plans: Make the initial and/or comprehensive nursing evaluation visit, determine the primary focus of care, develop the plan of care within state-specific guidelines, and submit accurate, complete, and timely documentation in collaboration with physician orders.
- Provide Patient Care: Perform patient comprehensive assessments at designated time points and develop the appropriate plan of care.
- Ensure Patient Eligibility: Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer source.
Requirements:
- Current RN Licensure: Current RN licensure in the state of practice.
- CPR Certification: Current CPR certification.
- Driver's License: Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
