Key Responsibilities:

1. Perform primary care assessments to identify individual patient needs for primary care services, ensuring valid consent and authorization for service. Collaborate with the Medical Director or other medical professionals as necessary.

2. Conduct medication reviews for patients as required. Document findings in writing and provide them to the designated site contact for inclusion in the patient's records.

3. Deliver services in accordance with LifeSource's site-specific protocols.

4. Upload documentation and billing records to the LifeSource EMR system at the end of each day and complete any other required end-of-day tasks as outlined in the Clinical Services Manual.

5. Foster open and positive communication with contract site staff, physicians ordering services, and authorized parties regarding patient care. Provide verbal and/or written instructions or feedback concerning medications and other relevant caregiver information.

6. Maintain knowledge of and adhere to LifeSource standards, policies, and procedures as indicated in the Clinical Services Manual. Stay informed of all legal and regulatory agency rules and guidelines, as well as professional ethical standards.

7. Conduct oneself in a professional manner at all times.

8. Undertake additional duties and responsibilities as assigned by LifeSource Management.

9. Participate in ongoing training to support certifications and licensures relevant to the position.

10. Uphold patient confidentiality at all times.

Qualifications:

  • Master's degree in Nursing or Physician Assistant Studies from an accredited institution and a current, valid license to practice in North Carolina. All necessary certifications must be current and valid.
  • Possess an active Federal DEA or the ability to obtain one.
  • Understanding of professional nursing theory, practices, and regulations to provide and evaluate patient care.
  • Familiarity with the rules and regulations governing healthcare.
  • Knowledge of the organization's policies and procedures.
  • Awareness of common safety hazards and precautions to maintain a safe work environment.

Required Skills:

  • Ability to gather and analyze physiological, socioeconomic, and emotional patient data.
  • Skill in accurately assessing patient issues and providing appropriate advice, intervention, or referral.
  • Proficiency in documenting clinical services.
  • Ability to exercise a high degree of initiative, judgment, and discretion.

Essential Abilities:

  • Ability to communicate effectively and maintain productive working relationships with patients, medical staff, and the public.
  • Ability to respond calmly and effectively in emergency situations.
  • Ability to interpret, adapt, and apply guidelines and procedures.
  • Ability to collaborate with all members of the healthcare team.
  • Ability to make responsible decisions within the scope of a dependent medical practitioner.
  • Ability to evaluate and recommend continuous quality improvement initiatives.
  • Ability to manage confidential and sensitive information.

Physical and Mental Demands:

This role requires walking, sitting, and standing typical of a standard office environment. It may involve lifting and carrying, as well as working under stress.
