About the Role
At Hopscotch Primary Care, we prioritize teamwork to address patient needs and deliver outstanding care. Our aim is to provide the kind of care that we would wish for ourselves or our family members, in the right environment and at the right time. The Hopscotch Care Team is tasked with delivering high-quality, personalized care to patients, which includes coordinating care among various healthcare providers and navigating the healthcare system effectively.
As a Senior Clinical Practitioner (Nurse Practitioner or Physician Assistant) on our care team, you will be instrumental in addressing patient needs, facilitating positive patient outcomes, and fostering a supportive patient experience. This position entails direct patient care and collaboration with a diverse interdisciplinary team to assess needs, customize care plans, and provide exceptional care. In addition to daily responsibilities, this role will contribute to enhancing the Hopscotch clinical model to improve patient care, outcomes, and experiences, while nurturing the culture of Hopscotch within our clinics and the community.
What You'll Do
As a Senior Clinical Practitioner, you will deliver comprehensive and interdisciplinary primary care with an emphasis on patient outcomes, quality metrics, and patient satisfaction. Specific responsibilities will encompass, but are not limited to:
- Providing comprehensive primary care services to a designated panel of patients, including in-person and telehealth consultations for routine and urgent primary care issues.
- Collaborating closely with physicians, community health workers, case managers, pharmacists, and others to achieve high-quality outcomes.
- Conducting thorough annual wellness visits and risk assessments.
- Performing phone triage and outreach as necessary.
- Potentially conducting home visits when required.
- Building trust with patients and caregivers, engaging them in discussions about care plans and wellness strategies.
- Proactively communicating across the care team and advocating for patients, understanding their needs, developing care plans, and delivering care to facilitate positive outcomes.
- Embracing a mindset of continuous improvement, adhering to evidence-based guidelines, and innovating care models that address real challenges faced by our patients.
- Meeting high standards for results on quality metrics and outcomes, including patient experience, STARS/HEDIS, and utilization.
Value-based Care and Clinical Excellence
- Exhibiting a commitment to value-based care and compliance, ensuring practices align with value-based interventions.
- Understanding and utilizing best practices and evidence-based approaches in care, collaborating with the Hopscotch team to integrate these practices and new strategies to enhance patient outcomes.

We are seeking a Senior Clinical Practitioner (Nurse Practitioner or Physician Assistant) who possesses:
- A minimum of two years' experience in a primary care specialty (internal medicine, geriatrics, family medicine).
- Experience with HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) coding, and HRA (Health Risk Assessments).
- Proficiency in navigating multiple software platforms, electronic health records, spreadsheet development, data processing, and managing various priorities effectively.
- An active, non-probationary state medical license.
- Authorization to work in the United States.
- Experience with the Medicare population, including in-home care and documentation compliance requirements.
- Experience caring for a panel of "at-risk" patients.
- Experience in teaching and training others, both formally and informally.
- Strong interpersonal and communication skills, both verbal and written.
- High emotional intelligence and collaboration skills when working with diverse groups.
- Proficiency in motivational interviewing, behavior change, health promotion, and coaching.
- Exceptional attention to detail, organizational, coordination, and planning skills.
- Solid problem-solving and critical thinking abilities.
- A proven capacity to quickly learn complex topics and apply knowledge effectively.
