Job Title: Weekend Physical Therapist Home Health

Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist to join our team in Burke, Virginia. As a Physical Therapist, you will play a vital role in providing high-quality care to patients in their homes.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to work with our interdisciplinary team. The ideal candidate will have a current license in the state of employment and a minimum of one year of experience as a PT in an acute setting. The Physical Therapist will be responsible for providing skilled physical therapy services to patients in their homes, educating patients and their families, and ensuring the safety of patients.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
  • Perform OASIS assessments to develop an individualized plan of care and make adjustments as needs change.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as exercise, massage, heat, water, etc.
  • Help decrease re-hospitalizations by front-loading visits for high-risk patients and teaching patients on preventative measures and making good decisions.
  • Counsel patients and families on the disease/injury and how to manage in all phases of physical therapy.
  • Instruct patients and families on proper use of equipment, such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Oversee and supervise therapy services of patients provided by PTAs and when appropriate, nurse assistants.
  • Communicate with PTAs and nurse aides on supervisory visit results.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
Requirements:
  • Physical Therapist with a current license in the state of employment.
  • Minimum of one year of experience as a PT in an acute setting.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
