Job Summary:We are looking for a compassionate and experienced Physical Therapist to provide skilled services to patients in their homes. As a member of our team, you will be responsible for developing and implementing individualized plans of care, educating patients and their families, and ensuring the safety of our patients.
Responsibilities:- Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
- Observe, record, and report patient reactions, changes, and progress to the attending physician and other relevant staff members.
- Develop and implement individualized plans of care, taking into account the patient's medical history, physical abilities, and goals.
- Educate patients and their families on proper use of equipment, such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
- Supervise and oversee therapy services provided by Physical Therapist Assistants (PTAs) and nurse aides, as needed.
- Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination.
- Current license to practice as a Physical Therapist in the state of Virginia.
- Minimum of one year of experience as a Physical Therapist in an acute setting.
- Valid driver's license and auto insurance in your name as a driver.
- Ability to lift, bend, and move patients as needed.
