Job Summary

Mercy Health is seeking a highly skilled Registered Nurse to serve as a Clinical Coordinator in our Women's Services department at Clermont Hospital. As a key member of our team, you will play a vital role in ensuring the smooth coordination of patient care, from pre-operative preparation to post-operative recovery.

Key Responsibilities
  • Coordinate patient care episodes, including scheduling specialist appointments, completing patient-reported outcome forms, and ensuring completion of pre-operative H&P.
  • Assess and identify high-risk patients, arranging adequate resources and clinical pathways to optimize patient outcomes.
  • Develop and implement pre-operative educational classes, ensuring timely distribution of educational materials.
  • Guide patients through the healthcare system, removing barriers by assisting with access issues and developing relationships with service providers.
  • Participate in review of quality metrics, maximizing outcomes related to discharge disposition and readmission rates.
  • Act as a liaison between physicians, healthcare staff, and patients, ensuring a successful episode of care.
  • Follow up with patients and initiate yearly follow-ups as necessary for final completion of patient-reported outcome measures.
Requirements
  • Minimum of two years of previous experience in a clinical specialty.
  • Ability to effectively communicate with diverse audiences.
  • Strong organization and detail orientation skills.
  • Prior experience working in protocol/pathway-driven environments.
  • Strong critical thinking skills.
