Job Overview

The Clinical Care Coordination Manager plays a crucial role in overseeing both the clinical and administrative functions of the care coordination team. This position is integral in the recruitment, onboarding, and continuous professional development of care coordination personnel. The manager will ensure that the team delivers a comprehensive range of services tailored to meet the mental health needs of clients, including skills training for clients and parents, connecting families with community resources, and leading treatment teams.

Reporting Structure:

Reports to: Care Coordination Program Director

Supervises: Care Coordinators

Key Responsibilities:

Supervision

  • Oversee daily operations and service delivery by care coordination staff in various settings including home, school, and community.
  • Provide clinical guidance and support to staff, ensuring adherence to established standards and promoting excellence in practice.
  • Participate in the recruitment, selection, training, and evaluation of care coordination personnel.
  • Facilitate orientation and ongoing training for new employees to ensure clinical competence and compliance with agency standards.

Administrative Duties

  • Administer oversight of staff to ensure compliance with operational standards and regulations.
  • Conduct regular reviews of client documentation to maintain adherence to regulatory requirements.
  • Assist staff in managing their caseloads and administrative responsibilities effectively.

Collaboration

  • Foster professional relationships within the organization and with external partners to enhance service delivery.
  • Encourage effective communication and a customer service-oriented approach within the team and the broader community.
  • Ensure collaboration among treatment providers to coordinate care for clients and their families.

Client Services

  • Provide direct assessment and care coordination services as needed, including supporting staff with client intakes and progress evaluations.
  • Develop transition plans for clients being discharged from services to ensure continuity of care.

Quality Assurance

  • Monitor service delivery to ensure compliance with best practice models and quality indicators.
  • Work with the program director to implement performance improvement initiatives as necessary.
  • Encourage staff participation in professional development opportunities to enhance skills and maintain licensure.

Financial Oversight

  • Assist in the preparation of annual budgets and ensure adherence to fiscal targets.
  • Support staff in understanding budgetary processes and maintaining sound financial practices.
  • Collaborate with the program director on program development and expansion efforts.

Qualifications:

This role requires a Master's degree in Social Work, Counseling, Psychology, or a related field. A minimum of three years of post-graduate experience and relevant licensure in the State of Ohio (LSW, LPC, MFT) is required, with independent licensure preferred. Clinical supervisory experience is highly desirable. A valid driver's license and a reliable vehicle are necessary for this position.

Work Environment:

The position involves working in a variety of settings, including office, school, and mental health environments. The role may involve exposure to challenging behaviors and various physical conditions.

Physical Requirements:

Employees must be able to perform essential functions, which may include sitting, standing, walking, and lifting up to 50 pounds. Safe operation of a vehicle in varying weather conditions is required.
